HEALTH PROGRAMS MEMBER AGREEMENT

By accepting the terms and conditions and becoming a member in this program, you are authorizing AMACORE GROUP, INC. to bill your credit card or checking account for this program. This charge shall remain in force until you notify AMACORE GROUP, INC. in writing of its cancellation. By becoming a member in this program, you are agreeing to the terms and conditions herein. This membership will automatically renew at the end of your membership term on a monthly basis, and your credit card or checking account will be automatically debited for your monthly fees. If you wish to change your payment method, please contact AMACORE GROUP, INC. at the number indicated.

TERMINATION: Discount Medical Provider Organization reserves the right to terminate the Membership of any Member or Member's dependents or deny eligibility to any person for any reason, including for a Member's failure or refusal to pay a participating provider of service, or for nonpayment of membership fees payable to AMACORE GROUP, INC. when due.

CANCELLATION POLICY: Please call the AMACORE GROUP, INC. to terminate or cancel your membership and automatically revoke your permission to continue any further payments. This membership can be cancelled for non payment.

REFUND POLICY: If you cancel for any reason within the first thirty (30) days of membership, you will receive a full refund. The one-time fee as disclosed is non-refundable.

DESCRIPTION OF SERVICES: Please refer to your fulfillment materials for a full description of services.

ADD OR UPDATE INFORMATION: If you would like to update information for you or your dependants, please call Billing Services at (866) 915-1153.

COMPLAINT PROCEDURE: If you would like to file a complaint, please call Member Services at (866) 915-1153.

• THIS PLAN IS NOT A HEALTH INSURANCE POLICY.
• This plan provides discounts at certain health care providers for medical services.
• This plan does not make payments directly to the providers of medical services.
• The plan member is obligated to pay for all health care service but will receive a discount from those health care providers who have contracted with this discount plan organization.

  LIMITATIONS, EXCLUSIONS & EXCEPTIONS

Neither the AMACORE GROUP, INC., the Association (if this program is sponsored by an association) or Administrator are licensed insurers, health maintenance organizations, or underwriters of health care services. No portion of any provider's fees will be reimbursed or otherwise be paid by the AMACORE GROUP, INC. or the Administrator.

DISCOUNT MEDICAL BENEFITS ARE NOT INSURANCE: This membership includes a discount medical benefits program that provides members with access to discounts on medical services by offering reduced rates through medical and health networks. The discount medical benefits are not insurance, nor should it be used in lieu of or to replace any existing health insurance. However, your membership may contain certain limited supplemental group coverage benefits. Members are responsible for payment in full at the time that medical or other services are provided by benefit providers. The AMACORE GROUP, INC., the Association (if applicable), the Administrator, their affiliates, agents, and sales representatives are not responsible for payment of any kind on behalf of members. The benefit providers and Administrators have no liability for providing or guaranteeing service or the quality of service rendered.

DISCOUNTS: This program makes it possible for members to receive reduced rates on various products and services through participating providers. Discounts received cannot be used in conjunction with any other kind of rate reduction mechanisms. Discounts are based on the provider's standard fees. Actual discounts will vary depending on the place and the specific services or products purchased. From time to time, certain providers may offer products or services to the general public at prices lower than the discounted prices available through this program. In such event, members will be charged the lowest price.

PROVIDERS: It is the member's responsibility to confirm with Member Services that a provider is an active participant of the program prior to seeing that provider. The Administrator reserves the right to add, change, and/or eliminate any of the service providers included in membership benefits without prior written notice to the member. It is possible that a member may not receive a discount on health-related services if they are covered by medical or health insurance or if the services were initiated prior to the member's activation of this membership.

MEDICAL TREATMENT: This program, participating association (if applicable), member's employer, this program's affiliates, agents, Administrator, and sales representatives are not responsible for providing or failing to provide medical services, treatments, consultations, diagnosis, or advice to a member or to a member's dependents by any service provider. This program has no liability for providing or guaranteeing medical services or for the quality of services rendered. Selecting a service provider is the member's obligation, which shall be made at the member's sole discretion. Such decision shall not be based on the advice, credentials, or recommendation of this program, its affiliates, agents, Administrator, or sales representatives. This program does not practice medicine nor does it interfere, participate nor form part of the provider-doctor/member-patient relationship.

SERVICE AVAILABILITY: This program does not guarantee that the products, services or facilities are available in every geographic location.

Utah Residents: This contract is not protected by the Utah Life and Health Guaranty Association.
Kentucky Residents: This contract is not an insurance policy and is not protected by the Kentucky Life and Health Guaranty Association
New York Residents acknowledge that the savings offered through the network of providers is subject to the network contracts applicable to the laws of New York State.
Washington State Residents: Membership nor any limited supplemental insurance association group policies or their associated benefits are not available to Washington state residents.
